· Perform network and outreach duties in collaboration with local communities and organizations to identify HIV positive populations in need of medical and supportive services.
· Perform case management duties including:
o Assist with connecting persons served to medical care, insurance enrollment, medical appointments and other appropriate social, mental health and financial services.
o Perform client reassessments, at minimum, every six months per grant guidance and document appropriately per internal policies.
o Develop and provide appropriate supportive referrals in focused areas for clients in need of services including housing, financial, nutrition, and psychosocial supports as well as Medication Assisted Treatment (MAT) and Behavioral Health (BH) as needed.
o Provide transportation, interpretation, information and education and access assistance.
o Fulfill charting and data collection duties, including training and successful use of CareWare or other EHR.
o Conduct home visits and transport clients to and from appointments (i.e. Legal, immigration, social services, medical, etc.) as needed and/or assigned by supervisor.
· Attend community meetings to ensure client needs are represented.
· Establish and maintain a good rapport and harmonious relationships with staff, clients and other program-related individuals.
· Assist with ongoing management and operations of the BAMSI Community Food Pantry provided in partnership with the Greater Boston Food Bank (GBFB) including inventory, food ordering, pickup and delivery as needed.
· Attend meetings, conferences, and trainings to expand knowledge of HIV disease and related topics (STI’s, Hepatitis, substance abuse, harm reduction, etc.) and be current on new developments as required by supervisor or designee.
· Perform duties consistent with agency/program policies and procedures.
· Execute duties to reflect reasonable safety standards. Universal/standard precautions must be utilized, and training obtained in areas that constitute risk.
· Perform other related work duties as needed or as required by supervisor or designee.
