Responsibilities

General Statement of Duties: Coordinate, monitor and support the health care and physical needs of persons served; provide staff and persons served trainings. Maintain electronic and paper medical records.

 


Responsibilities:

Clinical Coordination:

  • Maintain an assigned caseload and act as case manager, as directed.
  • Administer medications and treatments as prescribed by the PCP, ensuring adherence to CARF, DDS, and Mass Health regulations.
  • Ensure annual updates of physical examinations, health care provider orders, etc. in order to meet established guidelines.
  • Maintain medication books, appropriate sections of member medical files, to ensure program compliance with all regulations.
  • Monitor persons served health status and document those findings in the members medical record at least monthly, and every six months as part of the interdisciplinary team review and more often if indicated.
  • Report changes in the members condition to the member’s PCP.
  • Provide oversite of therapy treatment as recommended by a licensed therapist and or PCP order.
  • Advise the program director and staff of any medical problems that may hinder a member’s progress or that affect participation in the program. In collaboration with the program director, communicate medical updates to residential providers and families.

Training and Supervision:

 

  • Participate in the recruitment process and make recommendations for the hiring of prospective employees.
  • Provide staff and persons served training in relevant areas (e.g., health, hygiene, safety, diet, medical protocols, safe lifting, VNS, etc.).
  • Provide supervision of daily operations in absence of the program director.

 

 

Direct Care Responsibilities:

 

  • Monitor member lunch breaks as assigned and ensure protocols and diets are followed.
  • Provide direct care as needed by assisting persons served with ADL care.
  • Provide physical support, assistance with lifting and transferring members who have physical challenges as needed.

 

Administrative Duties:

 

  • Ensure compliance with Mass Health regulations governing day habilitation services, CARF standards and DDS regulations.
  • Maintain effective working relationships with funding source personnel, internal and external provider staff, persons served and family members.
  • Ensure that member’s Day Hab Service Plans are coordinated with other health care professionals and documented.
  • Attend ISP reviews and all other required meetings as assigned.
  • Complete all required documentation in a timely manner.
  • Utilize BAMSI’s electronic health care system; update information as required.
  • Utilize BAMSI’s electronic human resource system.
  • Participate in the orientation of new referrals.

 

Other:

 

  • Ensure adequate medication and medical supplies are on hand (e.g., routine, PRN and stock medications, gloves, gowns, etc.).
  • Provide assistance at other day programs as needed.
  • Perform other related work duties as needed or as assigned by supervisor or designee.
  • Perform duties to reflect program and agency policies and procedures.
  • Perform duties with a team approach.
  • Execute duties to reflect reasonable safety standards.  Universal/standard precautions must be utilized and training obtained in areas that constitute risk.

 

Qualifications

  • Licensed Nurse in the Commonwealth of Massachusetts.
  • Knowledge of rehabilitation activities, ISP development, CARF regulations, DDS philosophy, and behavior management experience desired.
  • Must possess a valid driver’s license in state of residence.
  • A demonstrated ability to make appropriate, accurate judgments including handling of emergency situations, and the mental and physical ability to successfully perform job duties and responsibilities which include, but are not limited to, providing physical support to consumers having physical challenges such as lifting and transferring persons served as needed. Ability to lift, transfer, push/pull, maneuver, and reposition 25 pounds.
  • Basic computer literacy, including the ability to record data, use electronic time and attendance software and receive and send electronic mail, as needed, is required.
  • The ability to communicate effectively, both verbally and in writing with persons served, families, service providers and case managers.
  • Demonstrated ability to set limits, to solve problems independently, and to appropriately protect confidential information.
  • Demonstrated ability to monitor and report on a participant’s progress, challenges and status verbally or in writing as required.
