    The MCCN (Massachusetts Care Coordination Network) Associate Care Coordinator will

    assist a team of Care Coordinators working at Affiliated Partner and/or Material Subcontractor agencies in their respective region. The Associate Care Coordinator will support Care Coordinators in providing coordinated Long Term Services and Supports (LTSS) care management services as an integrated member of care management team to youth and adults in need of services. Massachusetts Care Coordination Network (MCCN) is a partnership of highly qualified health and human service agencies that provide integrated comprehensive care coordination to people who are enrolled in MassHealth that have complex lifelong needs. By coordinating care, MCCN takes the best resources available and matches supports to each person’s needs. MCCN provides the tools that help people build self-directed, fulfilling lives.

     

    • High School Diploma
    • Two years of relevent experience required.Working with individuals with complex LTSS needs a plus.
    • Ability to use Care Management Software to document and coordinate services.
    • Must possess a valid driver's license, satisfactory driving record and own means of transportation. Local travel is required for this position.
    • Ability to read and speak English. Fluency in other language especially Spanish, Portuguese, Cape Verdean Creole, and/or Haitian Creole preferred.

    Responsibilities

    General Statement of DutiesThe Associate Care Coordinator will assist a team of Care Coordinators working at Affiliated Partner and/or Material Subcontractor agencies in their respective region. The Associate Care Coordinator will support Care Coordinators in providing coordinated Long Term Services and Supports (LTSS) care management services as an integrated member of care management team to youth and adults in need of services.

     

    Responsibilities:

     

    1. Assist and provide administrative support to the LTSS Care Coordinators in the performance of service delivery to Assigned or Engaged Enrollee.
    2. Review Assignee list to verify and update contact information prior to outreach by Care Coordinators.
    3. Complete first attempt at outreach to assigned Members prior to Care Coordinator commencing outreach.
    4. Assist Care Coordinators with Enrolled Members who recently have become ineligible for Mass Health.
    5. Complete all required documentation in a timely manner.
    6. Ensure that individuals receiving services are treated with dignity and respect in accordance with Massachusetts Care Coordination Network (MCCN) Human Rights Policy.
    7. Perform all duties in accordance with agency’s polices and procedures.
    8. Strictly follow all agency Performance Standards.
    9. Perform other related work duties as needed or as assigned by supervisor.

    Qualifications

    • High School Diploma
    • Two years of relevant experience. Working with individuals with complex LTSS needs a plus.
    • Ability to use Care Management Software to document and coordinate services.
    • Must be able to perform each essential duty satisfactorily.
    • Strong interpersonal skills in terms of developing a working relationship with a variety of individuals in a variety of contexts. Ability to communicate effectively verbally and in writing.
    • Strong organizational skills with attention to detail, multi-tasking skills, prioritization skills, analytical skills, problem-solving skills and team skills.
    • Strongly prefer that a candidate will have a demonstrated understanding of and competence in serving culturally diverse populations.
    • Commitment to MCCN values and mission.
    • Ability to travel. Must possess a valid driver’s license in state of residence, satisfactory driving record and own means of transportation.
